Kelly Dean, RN, BSN works as a Public Health Nurse for PPHD.  She assists anyone in the Panhandle in finding a health care, dental or eye care provider.  The population she works with most are those who have or are potentially eligible for Medicaid and/or Kids Connection. 

Some of the ways in which Kelly can help you are:

• Help to find and establish long term medical, dental and vision homes
• Assist with application process for Medicaid and Kids Connection
• Provide education on accessing appropriate levels of medical care.
• Identify barriers, such as language and transportation, that may prevent people from obtaining healthcare services
• Offer information on community resources and assisting with referral as needed. 

The Public Health Nurse (PHN) program is contracted with Nebraska Health and Human Services System to provide community outreach and education to Medicaid and Kids Connection eligible families.  The PHN program has nurses state wide to assist health care providers with the following issues and concerns for Medicaid and Kids Connection patients.

• Missed or late appointments
• Failure to present current Medicaid card at appointments
• Failure to comply or follow up with recommended medical, dental or vision care
• Inappropriate use of the ER
• Lack of health care coverage.
