Public Health Outreach Nurse/Educator Program
Mission-To improve the health status of Nebraska’s rural citizens by connecting families with physicians, dentists, optometrists, Health and Human Services, and other community organizations.

The Public Health Outreach Nursing program is contracted with Nebraska Health and Human Services System to provide community outreach and education to Medicaid and Kids Connection eligible families. The Public Health Outreach Nurse program has public health nurses available statewide to assist healthcare providers with the following issues and concerns for Medicaid and eligible Kids Connection patients.
Missed or late appointments.
Failure to present current Medicaid or Kids Connection card at appointments.
Failure to comply or follow up with recommended medical, dental, or vision care.
Inappropriate use of the
emergency
room.
Transportation barriers.
Lack of healthcare coverage.
The Public Health Outreach Nurse will assist eligible or potentially eligible Medicaid or Kids Connection individuals by:
Helping to find medical, dental, or vision homes.
Assisting with the application process.
Providing education on accessing appropriate levels of medical care.
Identifying barriers, such as transportation and language, that prevent the family from obtaining healthcare services, and
Offering information on community resources and assisting with referrals as needed.
Child Care Health Consultant
Every Childcare provider in the Panhandle has access to a child care consultant who can provide consultation and technical assistance on issues regarding:
Nutrition
Playground Safety
Injury Prevention
Oral Health
SIDS and Shaken Baby
Child Abuse and Neglect
Infectious Diseases
Immunizations
Health Standards
Licensing Regulations
Policies and Procedures
Mental Health in Children
Medication Management
Quality in Early Childhood
Environmental Health
Children With Special Needs
What does a Child Care Health Consultant do?
Promote health by assisting childcare providers with:
Written policies
Food safety practices
Sanitation procedures
Play equipment inspections
Health record review
Illness and injury records
Be available to all staff to discuss health concerns.
Provide telephone consultation to child care providers as issues arise.
Educate children, their family members and child care providers about any health issues that arise.
Help identify and implement improvement plans.
Link
children, families, and child care providers with community health resources
e.g. health screenings and health insurance.
