Free Health Assessment
The assessment only takes about 15 minutes to complete, and your results are provided to you immediately!

This comprehensive assessment provides a "snapshot" of your health by taking into consideration several different aspects of your lifestyle and behaviors. Your nutrition, stress, physical activity, use of alcohol and tobacco, and body chemistry are some of the factors that are covered by the assessment.

The results of your assessment are easy to read and detailed.  Depending on your health, your results will be covered in a 6-13 page document that you can print out and keep for future reference.  Please call us at (308)487-3600 if you have any questions or experience any problems when taking the assessment.

A health risk assessment is an excellent way for you to gain an understanding or your overall health status. To make your assessment as easy as possible, you should be aware of the following points:

• Vital Measures
  In order to get the full benefit of this assessment, you should know your current height & weight, blood pressure, cholesterol (including HDL), and glucose (blood sugar).  If you do not have this information, contact your local healthcare provider, or Panhandle Public Health District for assistance.
  
  
• Username and Password
  You will first be prompted to enter the login ID-public and the password-health.  After entering this information, you will then be asked to create a unique username and password so you can log back into the system to access your personal health survey and report.  Please choose a username and password that you'll be able to remember.
  
  *An easy-to-remember username would be first initial and last name, or first name and last initial(s) -- minimum of 6 characters.  Examples:  bsmith, bobbys, bob123, or bobs12.

          *Passwords must also be at least 6 characters. An easy-to-remember password
           would be:  survey or survey10.

• Site Code
  Unless a specific site code has been provided to you, simply enter
  site code: 000
